Trinity Member Database Update

Trinity is transitioning our member database to a new system. During the transfer, we discovered that many of our records in our previous database were incomplete or inaccurate. To ensure that we have accurate details for everyone, we are collecting information for all adults associated with Trinity. Please fill out this form with your most up-to-date information.

A new form should be submitted for all adults (persons age 18+). If you are filling out the form for more than one adult, you will see links on the confirmation page to return to the form.
